As you are aware we no longer provide paper copies of the masterpack. All of our forms are available online.

New forms coming

A new set of forms will become mandatory from 1 June 09.

To help you prepare we have posted previews of the forms under:

• CLS > Forms > Previewing June update 

Please take some time to familiarise yourselves with them.

When will the new forms be ready?

We will publish the new forms on 1 June 09 at:

• CLS > Forms

If you have saved links in your favourites, you'll need to refresh these to make sure you have the new versions.

We'll publish a version list and detailed information about the changes alongside the new forms.

Payments on Account

Included in these forms is a completely new claim form for Payments on Account (POA 1) which was successfully piloted with a number of Providers earlier this year: 

• it is for claims that would have been made previously on the Claim 4 and Claim 4A
• it allows for up to 15 claims to be sent in on the same form covering multiple cases  

The feedback from the providers involved in the pilot confirmed that the new form was a big improvement and up to 85% quicker to complete. 

Both the new and old forms can continue to be used until October 2009 and should be submitted in the normal way.
