About this form

The Mediation Work Start Form (and Mediation Consolidated Work Report Form) are for contracted family mediation organisations to submit monthly claims.

Mediation services must complete these forms setting out any mediation contract work they have started and completed during that period in order to get paid.

Please also refer to the 'Guidance for Reporting Work Under the New Family Mediation Contracts' when completing this form.
