Where to send this form

• If you work in the Nottingham region then send your form to the Nottingham office.
• If you work in the Birmingham region then send your form to the South Tyneside office.
• If you work in the Cambridge region then send your form to the Nottingham office.
• If you work in any of the above regions and your claim is in regards to Mental Health then send your form to the Liverpool office.

• If you work in the Wales region then send your form to the Nottingham office.
• If you work in the Wales region and your claim is in regards to Mental Health then send your form to the Liverpool office.

• If you work in the London region then send your form to the South Tyneside office.
• If you work in the London region and your claim is in regards to Mental Health then send your form to the Liverpool office.
