The TFF Forms A-E are:

• Applicable for all TFF exceptional cases for both voluntary and mandatrory schemes. This form should be submitted by 10 May each year.

• If you work in the Nottingham region then send your form to the Liverpool office.

• If you work in the London region then send your form to the Liverpool office.
