The L31 form is an application for a Legal Aid Certificate for:

financial information about an applicant’s partnership or self employed work. (Note: This form is not part of the Forms Masterpack but is listed here for your convenience.)

Providers need to fill this form in on behalf of their client. Please use black ink.

If you work in the Nottingham region then send your form to the Nottingham office.
If you work in the Birmingham region and your form is for Family work then send your form to the Birmingham office.
If you work in the Birmingham region and your form is for Non-Family work then send your form to the South Tyneside office.
If you work in the Cambridge region then send your form to the Chester office.

If you work in the Brighton region and your form is for Family work then send your form to the Nottingham office.
If you work in the Brighton region and your form is for Non-Family work then send your form to the Reading office.
If you work in the Bristol region then send your form to the Bristol office.
If you work in the Reading region and your form is for Family work then send your form to the South Tyneside office.
If you work in the Reading region and your form is for Non-Family work then send your form to the Reading office.

If you work in the Newcastle region then send your form to the South Tyneside office.
If you work in the Leeds region then send your form to the Liverpool office.
If you work in the Manchester region then send your form to the Manchester office.
If you work in the Liverpool region then send your form to the Chester office.
If you work in the Chester region then send your form to the Chester office.
