If you work in the Brighton region then send your form to the South Tyneside office.
If you work in the Bristol region then send your form to the Bristol office.
If you work in the Reading region then send your form to the South Tyneside.

If you work in the Newcastle region then send your form to the South Tyneside office.
If you work in the Leeds region then send your form to the Liverpool office.
If you work in the Manchester region and your work does not attract a statutory charge then send your form to the Chester office.
If you work in the Liverpool region and your work does attract a statutory charge then send your form to the Chester office.
If you work in the Chester region then send your form to the Chester office.
