Where to send this form

If you submit your Hardship Review application (CDS16) at the same time as your initial application (CDS14 and CDS15), both applications should be sent to the magistrates’ court where case will be heard.

As of the 1st February 2012 all Hardship Review applications (CDS16) must be submitted to the magistrates’ court that assessed the initial application.

Version 8 CDS14 and CDS15 forms can continue to be accepted for 3 months after the current stocks have been distributed by the forms printers.  We anticipate this cut off to will be at the beginning of May 2012, after which all new applications signed on or after this date must be made on Version 9.  For further information please see LSC updates in April.

Due to the differences between Version 8 and 9 forms they cannot be used together on the same application.
