The CDS5: Application for extension of upper limit form is used:
- To apply to the LSC to extend the upper costs limits specified in the General Criminal Contract
It must be submitted to the relevant processing centre. The form also contains the related appeal form.
Please use black ink. This form was last updated in November 2009.

Where to send this form
The Nottingham Office deals with all CDS5 applications.
We do not accept faxed applications unless it needs to be dealt with that day. You must get permisson to fax first by phone.
- If your application is urgent please ring before to discuss getting it prioritised.
- Our phone lines are open fom 10-12 & 3-5 Monday to Friday.
