Where to send this form

The Nottingham Office deals with all CDS4 applications now. The Chester office no longer processes any crime work so do not send your applications there.

We do not accept faxed applications unless it is a second post-mortem. You must get permisson to fax first by phone.
If your application is urgent please ring before to discuss getting it prioritised.
Our phone lines are open fom 10-12 & 3-5 Monday to Friday.
The national target for CDS4s is 5 working days. Please remember this for any urgent applications.
