The CDS10: Application for review of refusal or withdrawal of Advocacy Assistance or Representation form should be used in the following circumstances:

The client will complete this form where a provider has refused or withdrawn a grant of Advocacy Assistance. We will use this form when we refuse to grant representation. The completed form should be submitted to the LSC for consideration by the Funding Review Committee.
